I hereby authorize the Repairs outlined in my insurer’s Estimate of Record to be done by Auto Craft Body & Paint Company (”Auto Craft“). ”Repairs“ includes parts, labor and storage charges. I understand and agree that storage of $55/day is charged from the date my vehicle is delivered to Auto Craft and including the date that is removed from Auto Craft’s property.

I understand that Auto Craft does not stock parts for repairs, but instead orders them through manufacturer’s dealers. Dealer’s invoices may contain terms that limit the return of and refund for parts in the event that I or my insurer direct Auto Craft to stop work on my vehicle.* I acknowledge and agree that I, nor Auto Craft, will pay for all parts, including non-returnable parts and re-stocking fees for returned parts.

I further agree that Auto Craft shall no be responsible for any damage to my vehicle or my property left in it in the case of fire, flood, theft or other cause beyond Auto Craft’s reasonable control. Auto Craft employees are permitted to operate my vehicle on the streets, highways or elsewhere or elsewhere for the purpose of testing and inspecting it.

I acknowledge that an express mechanic’s lien shall exist on my vehicle to secure payment of the costs of parts and repairs and that an express garage keeper’s lien shall exist on my vehicle to secure the payment of storage. In the event of non payment by me, Auto Craft shall, in addition to its rights to enforce its liens, also be entitled to collect attorney’s fees associated with the costs of enforcement and collection of repair, parts and storage payment owed. I understand that MY CAR WILL NOT BE RELEASED UNTIL AUTO CRAFT HAS RECEIVED PAYMENT IN FULL FOR ALL REPAIRS, PARTS, STORAGE AND ANY ADDITIONAL SUPPLEMENTAL DAMAGE REPAIR CHARGES.

I hereby appoint Auto Craft to accept on my behalf any and all checks, drafts or bills of exchange, and to endorse all such checks, drafts or bills of exchange for deposit to Auto Craft’s account for credit to my account for repairs on my vehicle upon my acceptance and Auto Craft’s release of my vehicle. I understand that deductible and insurance check or proof of loss must be received before my vehicle will be released. In the event that payment from my insurance company is not made to Auto Craft directly, I agree that I will be responsible for payment of any unpaid invoices.

* Dealers may exclude returns on Special Order Parts, electrical parts, or parts that are not in the original packaging. Returns on other parts may be limited to a 7 or 14 day time period. Auto Craft does not guaranty that it will be able to schedule work or complete requests for supplemental work within the dealer’s allotted return window.
